1. Immunizations:
Please ensure your child’s immunization records are up to date and submitted to the school at the time of registration. They should have the following:

2. Physical Exam
The required physical exam shall be done no more than 365 days prior to the first day of school and submitted at the time of registration or before the first day of school. The form must be signed, stamped, and dated by your child's physician.
